Nurse Care Coordinator

The Registered Nurse - Patient Care Specialist is responsible for providing high-quality patient care and collaborating with the healthcare team to ensure optimal patient outcomes.
Key Responsibilities- Deliver direct and indirect patient care, including health maintenance, teaching, and counseling.
- Collaborate with physicians and other healthcare professionals to develop and implement patient care plans.
- Effectively delegate patient care activities to colleagues with the appropriate knowledge and skill level.
- Provide patient education and support to promote health and well-being.
- Participate in quality improvement initiatives to enhance patient care and safety.
- Associate's degree in Nursing required.
- Current Registered Nurse licensure required.
- Basic Life Support certification required.
- Advanced Cardiac Life Support certification required.
- 1-2 years of experience in a healthcare setting preferred.
- Previous experience in a patient care setting.
- Strong communication and interpersonal skills.
- Ability to work effectively in a team environment.
- Decision-making skills to prioritize patient care activities.
- Problem-solving skills to address complex patient care issues.
- Independence to work autonomously in a fast-paced environment.
- Written and verbal communication skills to effectively communicate with patients, families, and healthcare teams.
- Knowledge of patient care principles and practices to provide high-quality care.
This role requires the ability to stand and/or move around for the majority of the shift, with occasional lifting of up to 50 pounds.
