We are seeking a positive Licensed Practical Nurse (LPN) to play a vital part in our mission.

Key Responsibilities
  1. Support our site Monday through Friday, on-site at our Newton office, to assess appropriate levels of care and patient eligibility for hospice admission.
  2. Establish and maintain positive working relationships with current and potential referral sources.
  3. Assign referrals to each location and communicate schedules to the Director of Clinical Services promptly for admissions.
  4. Inform and consult with the Director of Clinical Services on all patient, personnel, and service management needs and challenges.
  5. Ensure confidentiality of all patient, physician, staff, and proprietary information.
  6. Collate and report referral statistics, including key customer referral trends.
  7. Conduct initial chart audits.
  8. Perform additional related duties as needed.

Requirements:

  • Associate degree or equivalent knowledge and experience, preferably in the hospice/healthcare field
  • Excellent verbal and written communication skills required, including telephone etiquette and negotiation skills
  • Proficient in Microsoft Office
  • Positive and professional attitude
