
Hospice Nurse

At West River Care, we're dedicated to providing exceptional home health care services to individuals in need. As a Hospice Nurse, you'll play a vital role in delivering compassionate care to patients and their families.
Key Responsibilities:- Conduct comprehensive assessments of patients and families to determine their care needs.
- Develop and implement personalized care plans that address the physical, emotional, and spiritual needs of patients.
- Provide skilled nursing care, including medication management, wound care, and symptom management.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Document patient progress and maintain accurate records.
- Graduate of an accredited nursing program.
- Current licensure in the state and CPR certification.
- 1-2 years of recent acute care experience and 1-2 years of recent home care experience.
- Bachelor's degree preferred.
- Excellent observation, verbal, and written communication skills.
