About This Role

We are seeking a highly skilled Social Worker to join our team at Methodist Le Bonheur Healthcare. As a Social Worker, you will play a critical role in coordinating patient-centered care and service delivery to facilitate optimal transitions and progression in care.

Key Responsibilities
  • Coordinate patient-centered care and service delivery to facilitate optimal transitions and progression in care.
  • Collaborate with patient, caregiver, physician, and other members of the healthcare management team to ensure seamless care coordination.
  • Develop and implement effective transition plans to promote efficient utilization of resources and reduce readmission risks.
  • Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and optimal care outcomes.
  • Monitor and document patient progress, identifying areas for improvement and implementing strategies to address care gaps.
Requirements
  • Master's Degree in Social Work (MSW) from an accredited institution.
  • 5-7 years of experience in a licensed social work role, with 2 years in an inpatient/acute care setting.
  • Licensed Clinical Social Worker (LCSW) or Licensed Medical Social Worker (LMSW) certification.
  • ACM/CCM certification preferred.

  • Excellent interpersonal and communication skills, with the ability to work collaboratively with interdisciplinary teams.
  • Comprehensive knowledge of community resources and social determinants of health.
  • Ability to gather and analyze data to inform care decisions and improve patient outcomes.
  • Strong problem-solving and critical thinking skills, with the ability to adapt to changing situations.
