Job Summary

The Hospice Team Coordinator is a vital member of the Compassus team, responsible for ensuring the seamless delivery of high-quality hospice care services to patients. This role requires a unique blend of clinical knowledge, administrative skills, and exceptional communication abilities.

Key Responsibilities
  • Patient Data Management: Accurately enter and manage patient data, including referral intake information, patient demographics, and updates to patient records.
  • Admission Documentation: Ensure all admission documentation is complete, signed, dated, and includes the attending physician's certification of terminal illness and order to admit.
  • Record-Keeping and Reporting: Use the HCHB system to maintain and update patient records, generate reports to demonstrate regulatory compliance, and add coordination notes as needed.
  • Supply Chain Management: Monitor the status of durable medical equipment and medical supplies orders, and promptly inform the management team of any notable trends or issues.
  • Physician Order Processing: Monitor the timely return of physician orders and facilitate their processing to maintain efficient patient care.
  • Visit Record Management: Verify and upload paper-based visit records into the Electronic Medical Record (EMR) system and subsequently into HCHB for complete and accurate patient histories.
  • Document Organization: Ensure accurate and systematic organization of uploaded documents in patient charts for easy retrieval and reference.
  • Level of Care Verification: Confirm that the ordered level of care matches the billable level of care for each patient, ensuring accurate billing and reimbursement.
  • Decertification and Discharge: Collect decertification paperwork and upload it into patient records, notify relevant healthcare providers of patient discharge through coordination notes.
  • Interdisciplinary Team Coordination: Facilitate coordination among members of the interdisciplinary team to ensure a holistic approach to patient care.
  • Communication and Customer Service: Act as a central point of contact for the hospice team, patients, and their families, facilitating clear and efficient communication.
  • Administrative Support: Handle incoming telephone calls, take accurate messages, and transfer calls to the appropriate personnel, maintaining a responsive and welcoming atmosphere.
  • Phone System Management: Manage the phone system and switch to the secondary system when necessary to ensure seamless communication.
  • Pre- and Post-IDT Activities: Assist the Hospice Director of Clinical Services with pre- and post-IDT activities, including preparing agendas and updating plans of care.
  • Facility Preparation: Prepare binders for patients in facilities, ensuring that essential information is organized and readily available.
Requirements
  • Education: High school diploma or GED required.
  • Experience: At least two (2) years of experience with medical records, medical office, or business office support highly preferred.
  • Skills: Strong written and verbal communication skills, ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations, ability to write reports, business correspondence, and procedure manuals, ability to effectively present information and respond to questions from leaders, team members, investors, and external parties.
