The Community Support Specialist is an integral member of the Community Support Team (CST) dedicated to enhancing the well-being of individuals through various support programs and care initiatives. This role is focused on providing comprehensive community-based rehabilitation services that include direct treatment, restorative interventions, and case management.

Key Responsibilities:

  • Engage effectively and respectfully with clients in their residences and community settings;
  • Encourage and foster relationships with family and friends, promoting a supportive family environment within the home and community, while enhancing the use of natural supports for community stabilization;
  • Build and maintain positive working relationships with community partners, including but not limited to social workers, instructors, day program staff, and court personnel;
  • Facilitate community-based opportunities aligned with client preferences;
  • Contribute positively during team meetings and special events;
  • Pursue additional relevant training based on personal interests or recommendations from management;
  • Assist in the orientation and training of new team members to ensure competent and confident staff, thereby ensuring the health, safety, and comfort of the individuals served;
  • Provide psychoeducation as outlined in the Individualized Support Plan;
  • Support clients during crisis situations by implementing de-escalation techniques;
  • Assist clients in attending professional appointments and developing life skills, independence, and rehabilitation as necessary according to their Individualized Support Plan;
  • Ensure connections to community services and resources;
  • Support the Team Leader in behavioral and substance abuse interventions;
  • Contribute to the development of relapse prevention and disease management strategies;
  • Participate in the initial creation, implementation, and ongoing revision of the Individualized Support Plan;
  • Communicate client progress and the effectiveness of strategies and interventions to the Team Leader as specified in the Individualized Support Plan;
  • Organize and engage in recreational and social activities with clients based on their interests;
  • Maintain accurate records and documentation for each individual, reporting any unusual or critical incidents in a timely and professional manner;
  • Familiarize oneself with medications utilized by clients and adhere to policies regarding medication administration;
  • Document all billable encounters in the electronic medical record system within the required timeframe, ensuring any corrections are made promptly;
  • Perform other duties as assigned by the Team Leader, Program Coordinator, or Clinical Director that relate to achieving performance objectives;

Supervisory Responsibilities:

This position does not include supervisory responsibilities.

Qualifications:

Education and Experience:

  • At least one year of experience supporting adults with behavioral or mental health challenges and/or substance abuse issues;
  • High school diploma, GED, or equivalent;

Required Skills and Abilities:

  • Ability to write clearly and legibly;
  • Flexible scheduling to accommodate client needs;
  • Knowledge of behavior management techniques, substance abuse counseling/support, and recreational leadership;
  • Ability to provide guidance and training in life skills;
  • Ability to implement Individualized Treatment Plans and monitor progress towards goals;
  • Effective communication skills, both oral and written;
  • A positive and nurturing demeanor that recognizes the importance of home and quality of life enhancement;

Certificates, Licenses, and Registrations:

  • Valid, unrestricted driver's license and proof of insurance;

Work Environment:

This role primarily involves working in clients' homes and community settings, which may include exposure to cigarette smoke, domestic animals, household pests, and varying heating/cooling conditions.

While performing the duties of this job, the employee is regularly required to stand, walk, use hands to handle objects, talk, and hear. The employee must be able to lift and/or move up to 20 pounds and occasionally lift and/or move up to 50 pounds. Specific vision abilities required for this role include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. The employee must be able to drive and operate a personal vehicle safely, adhering to all applicable state and traffic laws.
