About the Role

Southeastern Integrated Care LLC is seeking a highly skilled and compassionate Community Support Specialist to join our dynamic team. As a Community Support Specialist, you will play a vital role in providing community-based mental health and substance abuse rehabilitative services and supports to individuals with complex and extensive treatment needs.

Key Responsibilities
  • Provide direct and indirect interventions to support individuals in achieving rehabilitative and recovery goals.
  • Develop and implement Person-Centered Plans to ensure the individual's clinical needs are met.
  • Assist with crisis interventions and provide psycho-education as indicated in the Person-Centered Plan.
  • Coordinate services with community and natural supports, including medical and non-medical providers.
  • Monitor and document the status of the recipient's progress and the effectiveness of the strategies and interventions outlined in the Person-Centered Plan.
Requirements
  • Minimum of 1 year of documented experience with the adult MH/SA population.
  • Valid NC driver's license and personal vehicle insurance coverage.
  • Bachelor's degree in a human service field or related field.
