As a member of our Community Support Team, you will play a crucial role in helping clients achieve their recovery goals and improve their overall well-being. Our team consists of highly trained professionals who are passionate about making a difference in the field of behavioral health.

Position Overview

The Community Support Coordinator will work within a collaborative team of four professionals, focusing on providing essential community support services. This role involves developing and implementing Person-Centered Plans tailored to meet the unique needs of individuals with mental health and substance use challenges. Services are delivered in various community settings, ensuring accessibility and support for clients.

Key Responsibilities

  1. Deliver psycho-educational support as outlined in the Person-Centered Plan.
  2. Assist clients during crisis situations, providing immediate interventions.
  3. Develop and maintain the Person-Centered Plan, ensuring its effective implementation.
  4. Collaborate with the Team Leader to address behavioral and substance use treatment needs.
  5. Facilitate the development of relapse prevention strategies and link clients to appropriate services.
  6. Coordinate service delivery in alignment with defined service specifications.
  7. Provide training to staff on the specific needs of the adult population served.
  8. Participate in a 24/7 on-call system to support clients and their support networks.
  9. Engage with medical and non-medical providers to enhance person-centered planning.
  10. Monitor client progress and document the effectiveness of interventions.
  11. Manage budgetary considerations related to program operations.
  12. Support clients in transitioning to other services as needed.

Qualifications

To be considered for this role, candidates must possess a minimum of one year of documented experience working with adults in the mental health and substance use population. A qualified professional must hold the necessary licenses or degrees as defined by the regulatory standards.
