Job Description

Job Title: Community Support Specialist

Job Summary: We are seeking a highly skilled and compassionate Community Support Specialist to join our team. As a Community Support Specialist, you will play a vital role in providing community-based mental health and substance abuse rehabilitative services and supports to adults with complex and extensive treatment needs.

Key Responsibilities:

  • Assess client needs and develop comprehensive service plans to promote recovery, symptom reduction, and increased coping skills.
  • Provide direct and indirect interventions, including psycho-education, crisis interventions, and behavioral and substance abuse treatment.
  • Collaborate with the Community Support Team Leader, paraprofessional and associate professional staff to ensure seamless service delivery.
  • Monitor and document client progress, ensuring effective strategies and interventions are in place.
  • Participate in a first responder on-call system, providing 24/7/365 support to consumers and their natural support networks.
  • Consult with medical and non-medical providers, engage community and natural supports, and incorporate their input into the person-centered planning process.
  • Monitor budgetary costs and expenditures associated with business operations.
  • Coordinate service user transitions to other types of services.

Requirements:

  • Minimum 1 year of documented experience working with the adult MH/SA population.
  • Valid NC driver's license and personal vehicle insurance coverage.

Education and Certifications:

  • Bachelor's degree in a human service field or related field.
  • Valid licenses, certifications, or registrations as required by the governing board regulating a human service profession.
