Adult Mental Health Community-Based Case Manager

Zepf Center is seeking a dedicated and compassionate Adult Mental Health Community-Based Case Manager to join our team. As a key member of our community-based services, you will play a vital role in supporting individuals with mental health needs by providing extensive community support and coordinating services to ensure their needs are met.
The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and community agencies. A strong understanding of mental health principles and practices, as well as experience working with diverse populations, is essential.
Key Responsibilities- Evaluate client strengths and needs, collaborating with the client and relevant team members to develop a comprehensive care coordination and treatment plan.
- Track and assess client progress towards achieving goals, reassessing and adjusting the plan as needed.
- Maintain regular contact with clients, providing support and advocacy to ensure stability and progress.
- Link and refer clients to other services, programs, and agencies as needed, ensuring access to entitlements and community resources.
- Collaborate with supervisors and team members to ensure effective service delivery and coordination.
- High School Diploma or GED required, with demonstrated mental health experience; or Bachelor's degree in social work or three years of mental health experience preferred.
- CDCA certification required for SUD.
- Valid driver's license and acceptable driving record.
