Job Summary

We are seeking a skilled Community Based Case Manager for Adult Mental Health to join our team. As a key member of our staff, you will provide essential support to individuals struggling with mental health issues and substance abuse.

Key Responsibilities
  • Develop and implement individualized treatment plans to address mental health and substance abuse needs
  • Provide case management services, including counseling, advocacy, and support
  • Collaborate with healthcare providers, family members, and community resources to ensure comprehensive care
  • Conduct assessments, evaluations, and progress monitoring to inform treatment decisions
  • Maintain accurate and confidential records of client interactions and progress
Requirements
  • Bachelor's degree in a related field, such as social work, psychology, or counseling
  • Experience working with adults with mental health and substance abuse issues
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Valid driver's license and reliable transportation
