You will be responsible for evaluating client strengths and needs, collaborating with clients and team members to formulate care coordination and treatment strategies, and tracking client progress towards achieving goals.

As a Community-Based Case Manager, you will work closely with clients in their homes and community settings, providing support and guidance to help them achieve stability and independence. You will also be responsible for linking and referring clients to other services, programs, and agencies as needed, and advocating for clients to ensure their rights are respected.

Key Responsibilities:

  • Evaluate client strengths and needs, and develop comprehensive care plans in collaboration with clients and team members.
  • Provide support and guidance to clients in their homes and community settings, helping them achieve stability and independence.
  • Link and refer clients to other services, programs, and agencies as needed, and advocate for clients to ensure their rights are respected.
  • Track client progress towards achieving goals, and reassess and adjust care plans as needed.
  • Communicate effectively with clients, families, and community agencies to ensure seamless care coordination and treatment.

Requirements:

  • High School Diploma or GED required, with demonstrated mental health experience.
  • Bachelor's degree in social work or three years of mental health experience preferred.
  • CDCA certification required for SUD.
  • Valid driver's license and proof of auto insurance required.

Working Conditions:

This position involves extensive interaction with individuals and families, cooperation with multiple community organizations, and computer-based data entry and monitoring. You will be exposed to and need to deal with a range of emotionally charged issues in a compassionate and trauma-informed manner.
