Adult Mental Health Community Based Case Manager

Zepf Center is a leading provider of behavioral health and substance use disorder services in Northwest Ohio, serving the community for nearly 50 years. We are seeking a Full-Time Community-Based Case Manager to join our team and assist Adult Mental Health clients in achieving their goals.
Key Responsibilities- Evaluate client strengths and needs, collaborating with the client and relevant team members to develop a comprehensive care coordination and treatment strategy.
- Track and assess client progress towards achieving goals, reassessing and adjusting plans every 90 days as needed.
- Maintain regular contact with clients, providing support and advocacy to ensure stability and access to community resources.
- Link and refer clients to other services, programs, and agencies as needed, ensuring they obtain and retain applicable entitlements.
- Communicate with family members and significant others as indicated, serving as an advocate for clients and upholding a standard of patient engagement.
- High School Diploma or GED required, with demonstrated mental health experience; or Bachelor's degree in social work or three-years of mental health experience in behavioral health - preferred.
- CDCA certification required for SUD, with a valid driver's license and acceptable driving record.
This position involves extensive interaction with individuals and families, cooperation with multiple community organizations, and computer-based data entry and monitoring. The employee is expected to uphold a standard of patient engagement and work in a dynamic environment.
