Community Based Case Manager

Zepf Center is a leading provider of behavioral health and substance use disorder services in Northwest Ohio. We are seeking a Full-Time Case Manager to provide community support and coordination of services to individuals experiencing mental illness.
Job SummaryThe Case Manager will assist clients with developing short and long-term goals and acquiring community resources. They will participate in the development and coordination of comprehensive care coordination and treatment plans, advocate for clients, and provide assessment, linkage, and referral services.
Responsibilities- Assess client strengths and needs, and develop a comprehensive care coordination and treatment plan.
- Maintain contact with clients to ensure stability and provide support in achieving goals and outcomes.
- Link and/or refer clients to other services, programs, and agencies as needed.
- Assist clients in obtaining and retaining entitlements and follow up on same.
- Advocate for clients and communicate with family members and significant others as indicated.
- Confer with supervisor and other team members regarding clients and other agencies.
- May provide face-to-face contact with clients who are hospitalized to facilitate discharge planning and coordination of services.
- Assist clients in tracking and keeping appointments.
- Complete required paperwork in accordance with timelines and standards required by the agency, regulatory bodies, and payors.
- High School Diploma or GED required with demonstrated mental health experience; or
- Bachelor's degree in social work or three-years of mental health experience in behavioral health - preferred.
- CDCA certification required for SUD.
- Must have valid drivers license and be able to provide proof of auto insurance.
This is a full-time position with excellent wage, benefits, and retirement plan available. The employee will work in offices, participants' homes, and other community sites, and will have contact with a wide range of people, including some exhibiting anger and frustration under stressful conditions.
