Key Responsibilities
  • Evaluate client strengths and needs, collaborating with the client and relevant team members to develop a comprehensive care plan.
  • Provide ongoing support and advocacy for clients, ensuring they have access to necessary resources and services.
  • Link and refer clients to other services, programs, and agencies as needed, and assist with entitlements and follow-up.
  • Communicate effectively with clients, family members, and significant others, as indicated.
  • Participate in team meetings and conferences, sharing knowledge and expertise to ensure the best possible outcomes for clients.
Requirements
  • High School Diploma or GED required, with demonstrated mental health experience; or Bachelor's degree in social work or three years of mental health experience in behavioral health - preferred.
  • CDCA certification required for SUD.
  • Valid driver's license and acceptable driving record.
