Community Based Case Manager

Zepf Center is a leading provider of behavioral health and substance use disorder services in Northwest Ohio. We are seeking a Full-Time Case Manager to provide community support and coordination of services to individuals experiencing mental illness.
The Case Manager will assist clients in developing short and long-term goals and acquiring community resources. Services will be provided with respect and in accordance with clients' rights.
Key responsibilities include:
- Assessing client strengths and needs, and developing a comprehensive care coordination and treatment plan.
- Maintaining contact with clients to ensure stability and providing support in achieving goals and outcomes.
- Linking and referring clients to other services, programs, and agencies as needed.
- Advocating for clients and communicating with family members and significant others as indicated.
Requirements:
- High School Diploma or GED required with demonstrated mental health experience; or Bachelor's degree in social work or three years of mental health experience in behavioral health - preferred.
- CDCA certification required for SUD.
- Must have a valid driver's license and be able to provide proof of auto insurance.
This is a full-time position with excellent wage, benefits, and retirement plan available.
WORKING CONDITIONS
This job requires a high volume of individual and family contact, collaboration with various community agencies, and computer entry and tracking. Employee must be able to maintain a patient engagement standard.
Employee's work is conducted in offices, participants' homes, and other community sites. Employee has contact with a wide range of people, including some exhibiting anger and frustration under stressful conditions.
Must have a valid driver's license and acceptable driving record.
Zepf Center offers a comprehensive benefits package, including health, dental, and vision coverage, as well as a 403b retirement plan with a company match of up to 3% and a company contribution of 2% after one year of employment.
