Adult Mental Health Community Based Case Manager

Zepf Center is seeking a highly skilled and compassionate Community-Based Case Manager to join our team. As a key member of our mental health services team, you will work closely with adults who are coping with mental illness, providing extensive community support and coordinating services to ensure their needs are met.
The ideal candidate will have a strong background in social work or a related field, with experience in case management and community outreach. You will be responsible for developing and coordinating comprehensive care and treatment plans, advocating for clients, and facilitating assessments, linkages, and referrals.
This is a dynamic and rewarding role that requires cultural and racial diversity awareness and sensitivity. You will work in a fast-paced environment, interacting with a diverse group of individuals, some of whom may display anger and frustration due to stressful situations.
Key Responsibilities- Evaluate client strengths and requirements, collaborating with the client and relevant team members to formulate a detailed care coordination and treatment strategy.
- Track and assess client's advancement towards achieving goals, reassessing and adjusting the plans every 90 days.
- Maintain contact with the client as indicated in the treatment plan or as needed to ensure stability, communicating with and providing support to clients in achieving goals and outcomes.
- Link and/or refer clients to other services, programs, and agencies as needed, assisting in insuring clients obtain and retain applicable entitlements and following up on same.
- Serve as an advocate for clients, communicating with family members and significant others as indicated.
- Confers with supervisor and other team members regarding clients and, as appropriate, other agencies with emphasis on provision of most appropriate, effective and cost-efficient services.
- High School Diploma or GED required with demonstrated mental health experience; or Bachelor's degree in social work or three-years of mental health experience in behavioral health - preferred.
- CDCA certification required for SUD.
- Must have valid driver's license and be able to provide proof of auto insurance.
