Position Overview: We are looking for a dedicated and empathetic Case Manager to become a part of our Community Engagement Team within the PATH Program. This role focuses on providing extensive case management services to individuals facing homelessness, particularly those with significant mental health challenges.

Key Responsibilities:

  • Conduct eligibility screenings through various methods, including phone calls and in-person meetings.
  • Collaborate with clients to create personalized action plans.
  • Perform assessments to facilitate connections to housing, benefits, and additional resources.
  • Maintain accurate and timely documentation in client files.
  • Provide referrals to educational programs, counseling, legal services, and other community resources.
  • Follow up on referrals and advocate for clients as necessary.
  • Attend weekly meetings with supervisors to discuss client progress.
  • Stay informed about community resources by participating in workshops and meetings.
  • Complete required certifications within the first 90 days of employment.
  • Utilize and update client information in relevant databases.
  • Prepare necessary documentation using standard office software.
  • Perform additional duties as assigned by the supervisor.

Qualifications:

  • Bachelor's degree in social work or a related field.
  • At least one year of experience in social work or a similar role.
  • Ability to pass a Level Two DCF background screening.
  • Familiarity with local support services is preferred.
  • Strong verbal and written communication skills.
  • Proficient in computer applications, including Microsoft Office.
  • Ability to operate standard business equipment.
  • Willingness to learn and adapt to new documentation systems.
  • Completion of required training within the first 90 days of employment.

Physical Requirements:

  • Ability to conduct client assessments, which may involve prolonged periods of sitting.
  • Valid Florida State driver's license with a satisfactory driving record.
  • Ability to lift up to 15 lbs as needed.
