Position Title: Hepatitis C Patient Support Specialist

Location: Brooklyn, NY

Role Summary:

The Hepatitis C Patient Support Specialist plays a crucial role in facilitating access to care for clients diagnosed with HCV RNA. This position involves coordinating resources through collaborative efforts to ensure optimal health outcomes for patients.

Key Responsibilities:

  • Assist clients in navigating the healthcare system, ensuring they receive the necessary support and resources.
  • Establish and maintain relationships with healthcare providers and community organizations.
  • Conduct outreach initiatives at health fairs, community events, and other venues to promote health education and testing.
  • Develop and implement personalized Patient Navigation Care Plans, conducting thorough assessments of client needs.
  • Provide culturally sensitive health education and facilitate referrals to appropriate services.
  • Offer counseling on treatment adherence to support clients in managing complex medication regimens.
  • Maintain accurate patient records in compliance with relevant regulations and standards.
  • Ensure client information is managed in accordance with HIPAA guidelines.
  • Perform additional duties as required to support the mission of the organization.

Qualifications:

  • Bachelor's Degree in Social Work, Human Services, Psychology, or a related field is preferred.
  • Understanding of Hepatitis B and C.
  • A minimum of one year of experience in a healthcare setting.
  • Experience in case management, care coordination, or health coaching is highly desirable.
  • Strong written, verbal, and interpersonal communication skills.
  • Proficient in Microsoft Office and capable of managing data entry and reporting tasks.

Compensation:

Annual salary of $50,000, along with comprehensive benefits.
