Key Responsibilities:

  • Work collaboratively with our medical team to provide high-quality, patient-centered care.
  • Develop and implement patient navigation care plans, including assessments and referrals as needed.
  • Provide culturally sensitive health education and support to patients, promoting treatment adherence and positive health outcomes.
  • Conduct outreach and enrollment activities to connect patients with essential services and resources.
  • Maintain accurate and confidential patient records, adhering to HIPAA guidelines.
  • Perform other duties as assigned to support the delivery of exceptional patient care.

Requirements:

  • Bachelor's Degree in Social Work, Human Services, Psychology, or a related field.
  • Knowledge of Hepatitis B and C, with at least 1 year of previous healthcare experience.
  • Previous experience in case management, care coordination, health coaching, or similar services, preferably in a Health Home Care Management context.
  • Excellent written, verbal, and interpersonal skills, with proficiency in Microsoft Office and data entry.
