We are seeking a highly skilled Hepatitis C Health Educator/Patient Navigator to join our team at BMS Family Health and Wellness Centers. As a key member of our healthcare team, you will play a vital role in providing personalized care and support to our patients living with Hepatitis C.
Key Responsibilities:
- Work collaboratively with our medical team to provide high-quality, patient-centered care.
- Develop and implement patient navigation care plans, including assessments and referrals as needed.
- Provide culturally sensitive health education and support to patients, promoting treatment adherence and positive health outcomes.
- Conduct outreach and enrollment activities to connect patients with essential services and resources.
- Maintain accurate and confidential patient records, adhering to HIPAA guidelines.
- Perform other duties as assigned to support the delivery of exceptional patient care.
Requirements:
- Bachelor's Degree in Social Work, Human Services, Psychology, or a related field.
- Knowledge of Hepatitis B and C, with at least 1 year of previous healthcare experience.
- Previous experience in case management, care coordination, health coaching, or similar services, preferably in a Health Home Care Management context.
- Excellent written, verbal, and interpersonal skills, with proficiency in Microsoft Office and data entry.
