Position Overview
As a vital member of our team at BMS Family Health and Wellness Centers, the Hepatitis C Patient Advocate plays a crucial role in supporting individuals affected by Hepatitis C. This position focuses on providing essential resources and guidance to ensure optimal health outcomes for our clients.
Key Responsibilities
- Collaborate with healthcare professionals to deliver personalized, high-quality care.
- Develop and maintain a comprehensive Patient Navigation Care Plan, conducting thorough assessments.
- Offer culturally sensitive health education and facilitate necessary referrals.
- Provide counseling on treatment adherence to support clients in managing complex medication regimens.
- Connect clients with appropriate services that enhance health outcomes and promote self-sufficiency.
- Engage in outreach initiatives, including community health fairs and events, to raise awareness and facilitate enrollment.
- Assist clients in accessing supportive services and health insurance options.
- Ensure accurate maintenance of patient and program records in compliance with relevant standards and regulations.
- Uphold client confidentiality by managing information in accordance with HIPAA guidelines.
- Perform additional duties as required.
Qualifications
- A Bachelor’s Degree in Social Work, Human Services, Psychology, or a related field is preferred.
- Familiarity with Hepatitis B and C is essential.
- A minimum of one year of experience in healthcare is required.
- Prior experience in case management, care coordination, or health coaching is highly desirable, particularly within a Health Home Care Management context.
- Strong written, verbal, and interpersonal communication skills are necessary.
- Proficiency in Microsoft Office and the ability to manage data effectively are required.
