Job Summary

Housing Works is seeking a highly motivated and compassionate Patient Engagement and Prevention Specialist to join our Health Services division.

Key Responsibilities
  • Collaborate with the PCMH care team to coordinate and support PEP, PrEP, and Hepatitis C patients through the screening, assessment, care planning, and treatment processes.
  • Administer internal Hep C, PEP, and PrEP programs, ensuring seamless client navigation through all next steps.
  • Develop and implement culturally competent education on HIV prevention, Hepatitis C, and related available social services.
  • Perform assessments, including sexual history, mental health, substance abuse, and social determinants of health, and refer clients to appropriate supportive services.
  • Develop care plans to support treatment adherence and address social determinants of health.
  • Perform HIV, Hep C, and STI rapid testing, deliver results with sensitivity, and ensure clients move seamlessly through all next steps.
  • Collaborate with other departments to provide prevention education, encourage adherence to treatment, and complete referrals for other services.
  • Schedule case conferences, coordinating with medical and case management staff.
  • Perform outreach in the form of appointment reminders, no-show follow-up calls, and prescription updates.
  • Assist clients in obtaining insurance or overcoming insurance barriers to accessing medications.
  • Complete applications and collect and submit documentation as needed for medication Patient Assistance Programs and prior authorizations.
  • Work with pharmacy staff to coordinate delivery of prescriptions/refills.
  • Adhere to privacy and information security requirements (HIPAA) and other regulations.
  • Use culturally competent, sex-positive, non-stigmatizing language to discuss client history and care needs.
Requirements
  • Bachelor's degree in a health field or equivalent in experience preferred.
