Job Summary
We are seeking a highly skilled Hepatitis C Health Educator/Patient Navigator to join our team at BMS Family Health and Wellness Centers. As a key member of our healthcare team, you will play a vital role in providing personalized care and support to our patients living with Hepatitis C.
Key Responsibilities
- Work collaboratively with our medical team to provide high-quality, patient-centered care.
- Develop and implement individualized Patient Navigation Care Plans to ensure optimal patient outcomes.
- Provide culturally sensitive health education and referrals as needed to promote positive health outcomes.
- Offer treatment adherence counseling to support patients in managing complex medication regimens.
- Link patients to relevant services and resources within our system of care to promote self-sufficiency and treatment adherence.
- Conduct outreach and enrollment activities to engage with our target population.
- Provide referrals and assistance in accessing supportive services and health insurance.
- Maintain accurate and timely patient and program records in accordance with applicable standards and regulations.
- Manage client information in compliance with HIPAA guidelines.
Requirements
- Bachelor's Degree in Social Work, Human Services, Psychology, or a related field.
- Knowledge of Hepatitis B and C.
- At least one year of previous healthcare experience.
- Previous experience in case management, care coordination, health coaching, or similar services to a target population, preferably in a Health Home Care Management context.
- Excellent written, verbal, and interpersonal skills.
- Proficiency in Microsoft Office and ability to enter data and run reports.
