About the Job:

In this role, you will work closely with patients and their families to develop and implement personalized care plans that meet their unique needs. You will also provide education and support to help patients manage their symptoms and improve their quality of life.

Responsibilities:

  • Assess and evaluate patient care needs
  • Develop and implement personalized care plans
  • Provide education and support to patients and their families
  • Collaborate with healthcare teams to ensure seamless care delivery

Requirements:

  • 2 years of experience as a professional nurse
  • Current license to practice in the state of Ohio
  • Ability to operate a motor vehicle required

Estimated Salary: $75,000 - $95,000 per year
