Compassionate Hospice Nurse Coordinator

About Gentiva Hospice:
We are seeking an experienced RN Case Manager to provide exceptional patient care and support to our hospice patients and their families.
Key Responsibilities:
- Collaborate with interdisciplinary teams to ensure comprehensive patient care
- Provide education and support to patient caregivers
- Evaluate patient needs and develop personalized plans of care
- Maintain accurate and up-to-date clinical records
- Participate in quality assessment and performance improvement initiatives
Requirements:
- RN license in the state of employment (or eligibility to obtain)
- 1+ years of nursing experience in hospice, home health, or geriatric settings preferred
- Current driver's license and automobile insurance
- Current CPR certification
Estimated Salary: $65,000 - $85,000 per year
