Nursing Professional: End-of-Life Care Specialist

We are seeking a compassionate and skilled RN to join our team as an Inpatient Hospice Nurse. As a vital member of our interdisciplinary team, you will provide exceptional care to patients and their families during the end-of-life journey.

Job SummaryThis is a part-time night shift position located at The Ohio State Wexner Center. As an Inpatient Hospice Nurse, you will work closely with our interdisciplinary team to deliver comprehensive care to patients with serious illnesses. Your expertise will be essential in creating a supportive environment that promotes comfort, dignity, and peace of mind for our patients and their families.
Responsibilities- Care Coordination: Work collaboratively with our interdisciplinary team to develop and implement personalized care plans that address the physical, emotional, and spiritual needs of our patients.
- Nursing Expertise: Utilize your clinical skills and experience to provide high-quality care to patients with diverse medical needs.
- Patient Education: Educate patients and their families on disease management, medication use, and self-care techniques to promote independence and well-being.
- Documentation: Accurately document patient information, including progress notes, medication lists, and treatment plans, in our electronic health record system.
- Interdisciplinary Collaboration: Participate in regular care conferences, IDT meetings, and other collaborative activities to ensure seamless care coordination and communication among team members.
- Education: Graduate of an accredited nursing school; Associate of Science in Nursing (ASN); Bachelor of Science in Nursing (BSN) preferred.
- Licensure: Active Registered Nurse license in the State of Ohio without any board actions.
- Experience: Two years acute care nursing experience preferred.
- Certification: Certification in Hospice & Palliative Medicine (CHPN) preferred and available to Ohio's Hospice employees who wish to obtain certification.
