As a skilled RN Hospice House/Inpatient Care professional, you will play a vital role in our mission to make a meaningful difference in the lives of our patients and their families.
About the RoleWe are seeking a compassionate and experienced Registered Nurse (RN) to join our team at Ohio's Hospice. This part-time night shift position is located within the state-of-the-art facilities of The Ohio State Wexner Center.
Your primary responsibilities will include delivering high-quality patient care, collaborating with interdisciplinary teams, and documenting accurate information in electronic health records. You will also have the opportunity to teach, evaluate, and supervise care provided by Personal Care Specialists, Volunteers, and other caregivers.
Key Responsibilities- Plan and deliver individualized patient care utilizing the nursing process, ensuring assessment, planning, intervention, implementation, and evaluation are completed effectively.
- Collaborate with patients, families, and interdisciplinary team members to develop and implement comprehensive care plans.
- Assess and evaluate patient needs, including physical, spiritual, and psychosocial requirements, to ensure quality patient outcomes.
- Document accurate information in electronic health records, ensuring safe delivery of services and continuity of care.
- Supervise, teach, and evaluate care provided by Personal Care Specialists, Volunteers, and other caregivers.
To be successful in this role, you will need:
- A Graduate degree from an accredited nursing school.
- An active Registered Nurse license in the State of Ohio without any board actions.
- A minimum of two years' experience in acute care nursing, preferably in hospice or palliative care settings.
- Certification in Hospice & Palliative Medicine (CHPN) is highly preferred.
- Proficiency in computer skills, including electronic health record documentation.
- The ability to drive during nighttime hours and inclement weather conditions.
