Hospice Social Work Specialist

Overview
We are seeking a skilled Social Worker (MSW) or (BSW) to join our interdisciplinary team. The successful candidate will provide psychosocial support to patients and their families, utilizing their professional training and judgment to assess needs, goals, interventions, and services. This role is critical to our plan of care, ensuring that patients receive comprehensive support throughout their journey.
About You
To succeed in this role, you must possess a Master's or Bachelor's Degree in Social Work from a CSWE-accredited school, along with licensure as required by the state. One year of experience as a Social Worker in a healthcare or hospice setting is necessary; three to five years of experience is preferred. You should be knowledgeable about terminally ill patients and their families, with a deep understanding of hospice and the psychosocial dynamics of illness, loss, and death. Your ability to apply your knowledge, demonstrate sensitivity, and cope with stress will make you an invaluable asset to our team.
