We are seeking a compassionate and skilled Hospice Registered Nurse to join our team at Heartland Hospice. As a Hospice RN/Case Manager, you will play a vital role in providing personalized care to patients and their families during the most challenging times.

About the RoleThe Hospice RN/Case Manager will be responsible for:
- Providing direct patient care services, including assessments, medications, and treatments
- Serving as a liaison between the patient, family, and healthcare team
- Developing and implementing individualized care plans to meet the unique needs of each patient
- Collaborating with other healthcare professionals to ensure comprehensive care
- Maintaining accurate and timely records of patient interactions and care
To be successful in this role, you will need:
- A current RN license in the state of employment (or eligibility to obtain)
- 1+ year of experience in hospice, home health, ICU, geriatric, or oncology nursing
- Demonstrated ability to work independently and as part of a team
- Excellent communication and interpersonal skills
