End-of-Life Care Nurse Specialist

Salary InformationThe estimated annual salary for this role is $85,000 – $105,000, depending on experience and qualifications.
Job DescriptionThis End-of-Life Care Nurse Specialist will assume primary responsibility for patient and family caseloads, including assessment, eligibility determination, admission processing, planning, implementation, and evaluation phases of the nursing process. Key responsibilities include:
- Ongoing assessment and reassessment of patient needs and development of individualized care plans
- Collaboration with the interdisciplinary team to coordinate optimal care and ensure smooth transitions between healthcare settings
- Provision of regular visits to re-assess patient needs and update care plans as necessary
In addition, the ideal candidate will possess strong communication skills, be able to maintain accurate patient records, and demonstrate knowledge of quality assessment and assurance procedures.
Required Skills and QualificationsTo excel in this role, candidates must meet the following requirements:
- Graduate from an accredited registered nursing program
- Licensed as a registered nurse in the state of practice
- Current CPR and First Aid certification
- Valid driver's license and proof of insurance
