Hospice Liaison Specialist

Ambercare is passionate about providing patient-centered hospice care that maximizes resources at bedside while being operated locally. We are seeking a skilled RN Transitional Care Navigator to join our team and contribute to this mission.
About the RoleThis position will work collaboratively with the Hospice Liaison team and Admissions team to increase public awareness of hospice services and drive admissions. The successful candidate will serve as the first point of contact for hospitals, healthcare facilities, and other sources regarding referrals, and seek networking opportunities to provide education and develop relationships with new and existing referral sources.
The ideal candidate will have excellent communication skills, be knowledgeable of quality assessment and assurance procedures, and possess the ability to make independent decisions when circumstances warrant.
