Cancer Care Program Nurse Coordinator (Full Time, Day Shift)

Position Overview

The Cancer Care Program Nurse Coordinator is responsible for assessing, planning, implementing, and evaluating patient care in accordance with established organizational policies and procedures. This role serves as the primary liaison for patients and their families, ensuring a seamless experience throughout their treatment journey.

In collaboration with physicians and healthcare leaders, the coordinator acts as a vital link between patients, families, and various specialties within the healthcare system.

The position focuses on maintaining consistent communication with program patients to remove obstacles that may delay diagnosis and treatment, thereby facilitating comprehensive care across the healthcare continuum.

Key Responsibilities

  • Support patients diagnosed with hematological malignancies at our Cancer Center.
  • Possess a Bachelor’s Degree in Nursing (BSN).
  • Maintain an active Registered Nurse (RN) Licensure in the State of Florida or a Multi-State Enhanced Nursing License Compact (eNLC).
  • Hold a current American Heart Association BLS certification.
  • Have a minimum of three years of experience in Hematology/Oncology.

  • Master’s Degree in Nursing.
  • Specialty Certification in relevant fields.
  • Bilingual proficiency in English and Spanish.
  • Excellent interpersonal communication skills.
  • Strong analytical and problem-solving abilities.

  • Proficient in Microsoft Office applications.
  • Demonstrated clinical knowledge of healthcare systems and patient care processes.
Job-Specific Duties

  • Ensure the delivery of safe and efficient patient care by collaborating closely with physicians across specialties to coordinate care plans.
  • Facilitate the development of a collaborative and streamlined process for patient and family transitions while engaging with community physicians and related services.
  • Work alongside multidisciplinary teams to maintain and implement coordinated patient care, ensuring effective communication among all healthcare team members.
  • Implement clinical protocols to refer patients to appropriate specialists for diagnosis and treatment, assisting families in securing necessary referrals and appointments.
  • Collaborate with a multidisciplinary team to provide appropriate support based on clinical situations, serving as a clinical expert within the department.
  • Identify and mitigate barriers within the healthcare system to guarantee timely access to quality medical care.
  • Proactively enhance clinical processes to ensure an excellent patient experience, facilitating smooth transitions between inpatient and outpatient care.
  • Communicate effectively with referring physicians to address patient and physician needs, acting as a liaison to community organizations for ongoing support services.
  • Educate patients about their conditions, providing educational materials and developing care plans to enhance understanding and satisfaction.
  • Collaborate with marketing and outreach teams to inform referring physicians and facilities about available services.
  • Assist in screening and referring patients for eligibility in research clinical trials.
  • Act as a nurse liaison to facilitate patient management services, coordinating complex care from admission through outpatient follow-up.
  • Ensure effective communication between staff and physicians, proactively scheduling appointments and follow-ups, and serving as a resource for obtaining clinical information.
  • Utilize data to integrate evidence-based practices into patient care processes.
  • Maintain meticulous records of physician and patient inquiries, tracking outcomes related to communication timeliness and referral completions.
  • Disseminate new information throughout the institution and community, remaining informed about advancements in specialty care.
  • Promote a safe working environment by adhering to patient safety protocols and performing thorough hand-offs.
