Key Responsibilities:
  • Deliver comprehensive case management services and maintain ongoing communication with youth, families, and Juvenile Probation Officers under the supervision of a licensed mental health professional.
  • Conduct or coordinate all necessary mental health and substance abuse screenings and evaluations.
  • Prepare performance summaries, treatment plan reviews, aftercare plans, and discharge summaries.
  • Facilitate case management tasks, including preparing families for program visits and aftercare responsibilities.

Qualifications:
  • A Bachelor's degree from an accredited institution in counseling, social work, psychology, rehabilitation, special education, or a related human services field is required.
  • Two years of experience working with children facing serious emotional challenges or substance abuse issues. A master's degree in a relevant field may substitute for some experience.
  • Experience in a Medicaid provider setting is advantageous. Familiarity with group work techniques is a plus.
  • Ability to navigate the property and participate in youth activities, including transportation of youth or staff.
  • Must be free from communicable diseases as defined by state regulations.
  • Successful completion of a physical techniques evaluation and written certification exam is required.
