Key Responsibilities:
- Deliver comprehensive case management services, ensuring ongoing communication, evaluations, and assessments for youth, families, and Juvenile Probation Officers under the supervision of a licensed mental health professional.
- Conduct or facilitate all necessary mental health and substance abuse screenings and assessments.
- Prepare performance summaries, treatment plan reviews, aftercare/transition plans, and discharge summaries.
- Assist families in preparing for program visits, home leave experiences, family/group conferences, and aftercare responsibilities.

Qualifications:
- A Bachelor's degree from an accredited institution in counseling, social work, psychology, rehabilitation, special education, or a related human services field is required.
- A minimum of two years of experience working with children facing serious emotional disturbances or substance abuse issues. Candidates with a Master's degree in a relevant field may have preferred experience requirements.
- Familiarity with Medicaid provider environments is advantageous. Experience with group work techniques is a plus.
- Ability to navigate the property, engage in youth activities, and transport youth or staff as needed.
- Must be free of communicable diseases as defined by state regulations and capable of passing a physical techniques evaluation and written certification exam.
