Job Summary

We are seeking a highly skilled and compassionate RN MDS Coordinator to join our team. As a key member of our nursing staff, you will be responsible for developing and implementing care plans for each resident, ensuring that their unique needs are met and their quality of life is enhanced.

Key Responsibilities
  • Develop and implement care plans for each resident, in collaboration with the Director of Nursing and other healthcare professionals.
  • Conduct and coordinate the development and completion of the resident assessment, in accordance with State requirements and facility policies and procedures.
  • Use data from standardized assessments to evaluate the functional capabilities of each resident and inform care planning decisions.
  • Collaborate with the interdisciplinary team to ensure that care plans are aligned with the resident's goals and preferences.
  • Communicate effectively with residents, family members, and healthcare professionals to ensure that care plans are implemented and updated as needed.
Requirements
  • Nursing degree from an accredited college or university.
  • Current, unencumbered license to practice as a Registered Nurse in California.
  • Current knowledge and understanding of nursing and medical practices and procedures, as well as relevant laws, regulations, and guidelines that pertain to long-term care.
  • Ability to read, write, speak, and understand the English language.
  • Excellent communication, decision-making, and interpersonal skills.
  • Microsoft Office and computer proficiency, with experience with electronic records preferred.
