Job Description

We are seeking a skilled Clinical Intake and Patient Care Coordinator to join our team at NEXT CARE HOSPICE. As a licensed LVN or RN, you will play a crucial role in carrying out daily intake activities for our hospice agencies.

Key Responsibilities:

  • Handle assigned duties in a timely and efficient manner, ensuring seamless patient care and coordination.
  • Assist in the process of patient intake, including scheduling appointments and communicating with healthcare professionals.
  • Maintain strong relationships with Case Managers, Nursing Homes, Hospitals, Physicians, and other referral sources to ensure effective care coordination.
  • Coordinate ambulance transportation, order supplies and equipment, and facilitate bedside meetings as needed.
  • Communicate with hospitals, foundations, nursing and board and care homes, and provide Explanation of Hospice Benefits to patients and families.
  • Facilitate in-person or phone appointments with patients' families and physicians to ensure comprehensive care.

Qualifications:

  • Must be a licensed LVN or RN with previous experience in healthcare administration or related fields.
  • Familiarity with patient intake processes and ability to work with a variety of healthcare professionals and administrators.
  • Strong organizational and communication skills, with the ability to follow up on tasks and ensure timely completion.
