Job Summary

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a MDS Coordinator. In this role, you will be responsible for developing and implementing care plans for each resident, working closely with the Director of Nursing and other healthcare professionals to ensure the highest level of care.

Key Responsibilities
  • Develop and implement care plans for each resident, including conducting and coordinating the development and completion of the resident assessment in accordance with State requirements and facility policies and procedures.
  • Use data from standardized assessments to evaluate the functional capabilities of each patient and inform care planning decisions.
  • Collaborate with the Director of Nursing and other healthcare professionals to ensure seamless care coordination and communication.
  • Provide exceptional customer service and maintain positive relationships with residents, family members, and other stakeholders.
  • Stay up-to-date with relevant laws, regulations, and guidelines that pertain to long-term care and ensure compliance with all applicable standards.
Requirements
  • Nursing degree from an accredited college or university.
  • Current, unencumbered license to practice as a Registered Nurse in California.
  • Current knowledge and understanding of nursing and medical practices and procedures, as well as relevant laws, regulations, and guidelines that pertain to long-term care.
  • Excellent communication, decision-making, and interpersonal skills.
  • Microsoft Office and computer proficiency, with experience with electronic records preferred.
