Care Coordination Specialist

The Care Coordination Specialist plays a vital role in the management of patient care within a hospice setting. This position involves a variety of responsibilities aimed at ensuring that patients receive timely and effective support.
Key Responsibilities:- Communication: Handle incoming calls and provide assistance to hospice patients and their families regarding appointment scheduling and medication refills.
- Calendar Management: Oversee and maintain the schedules for LVNs, RNs, CHHAs, and on-call staff.
- Medical Coordination: Collaborate with healthcare professionals to arrange continuous care and PRN visits as necessary.
- Data Management: Input and update patient records in the electronic medical record system.
- Customer Service: Deliver exceptional service to patients, addressing inquiries and concerns promptly.
- Interdepartmental Collaboration: Work with various departments to coordinate patient care and scheduling needs, including physical therapy, podiatry, wound care, and laboratory services.
- Inventory Management: Order durable medical equipment (DME), maintain supply inventory, and implement cost-saving measures.
- Billing Support: Complete prior authorizations, verify benefits, and prepare charts for billing to ensure timely reimbursement.
- Minimum of one year of experience in hospice care is required.
- Bilingual proficiency is mandatory.
- Familiarity with electronic medical record systems, particularly Hospice MD, is advantageous.
- Strong verbal and written communication skills are essential.
- Excellent organizational abilities and attention to detail are required.
- Capability to multitask and prioritize effectively.
- Proficiency in computer applications, including Google Suite.
- Ability to work independently and see tasks through to completion.
