Position Overview:

The MDS Coordinator plays a crucial role under the guidance of the Director of Nursing. This position is responsible for creating individualized care plans for residents and overseeing the assessment process in compliance with state regulations and facility protocols. The MDS Coordinator utilizes standardized assessments to evaluate each resident's functional capabilities.

Key Responsibilities:

  • Develop and implement care plans tailored to each resident's needs.
  • Conduct and coordinate resident assessments in line with regulatory requirements.
  • Collaborate with nursing staff and other healthcare professionals to ensure optimal care delivery.

Qualifications:

  • Degree in Nursing from an accredited institution, with prior experience as an MDS Coordinator in a skilled nursing environment preferred.
  • Valid and unrestricted Registered Nurse license in California.
  • Strong understanding of nursing practices, medical procedures, and relevant regulations in long-term care.
  • Proficient in English, with excellent communication and interpersonal skills.
  • Ability to interact tactfully with residents, families, and staff.
  • Familiarity with Microsoft Office and electronic health records is advantageous.
  • Willingness to work collaboratively and supervise team members.
