We are looking for a dedicated Clinical Intake and Patient Care Coordinator to manage the daily intake processes for our Hospice services. This role is ideal for candidates with LVN or RN licenses, and experience in hospice intake is highly valued. Bilingual candidates, particularly those fluent in Spanish, are encouraged to apply. This position also includes performance incentives.


Key Responsibilities:
  • Execute duties assigned by the Director of Nursing promptly.
  • Support the patient intake and care coordination process.
  • Build and maintain strong relationships with Case Managers, Nursing Facilities, Hospitals, Physicians, and other referral sources.
  • Assist in organizing ambulance transportation, procuring necessary supplies and equipment, and conducting bedside meetings.
  • Communicate effectively with hospitals, foundations, nursing homes, and assist patients and families in understanding Hospice Benefits.
  • Facilitate appointments, either in-person or via phone, with patients' families and their physicians.
Qualifications:
  • Must possess a valid LVN or RN license.
  • Prior experience in healthcare administration or related fields is required; hospice experience is preferred.
  • Understanding of the patient intake process is essential.
  • Ability to collaborate with various healthcare professionals and administrators.
  • Excellent organizational skills are a must.
  • Strong communication and follow-up abilities are necessary.
