Position Overview

Nuvance Health is a prominent healthcare network offering a variety of hospital and outpatient services across multiple locations. Our facilities include well-known hospitals and specialized care centers, dedicated to providing exceptional medical services.

Role Summary:

The Clinical Coordinator is responsible for overseeing the continuity of care for patients within the Med-surg/Acute Rehabilitation unit. This role involves managing a skilled team that excels in critical thinking, clinical judgment, and effective communication with all healthcare professionals. The Clinical Coordinator will play a vital role in delivering clinical expertise and collaborating with the healthcare team to implement quality initiatives, enhance performance, and ensure compliance with regulatory standards.

Key Responsibilities:

  1. Collaborate with the healthcare team to formulate comprehensive care plans in partnership with patients, families, and providers to ensure safe and effective care.
  2. Monitor compliance with care plans, assess their effectiveness, track patient progress, and maintain open communication with the healthcare team.
  3. Encourage patient self-management through education and shared decision-making, advocating for patient needs.
  4. Develop and enforce policies and procedures that support optimal patient care.
  5. Contribute to the operational and capital budget planning.
  6. Assist in establishing departmental goals and quality metrics, including data collection and reporting to relevant stakeholders.
  7. Work closely with the Director and nursing staff to manage daily staffing needs and create a 6-week staffing schedule.
  8. Provide ongoing educational opportunities for staff, covering annual training, regulatory requirements, and patient safety protocols.
  9. Exhibit effective leadership and drive continuous improvement within departmental activities, either directly or through delegation.
  10. Maintain up-to-date professional knowledge and engage in continuous education.
  11. Demonstrate consistent and reliable attendance.
  12. Perform additional duties as necessary.

Qualifications:

A Bachelor of Science in Nursing (BSN) is preferred. Current certifications in BLS and ACLS are required.

Work Environment:

This position is based in a dynamic healthcare setting, requiring some manual dexterity and the ability to exert light physical effort. Exposure to various environmental factors may occur.
