Position Summary
Nuvance Health operates a comprehensive network of hospitals and outpatient facilities, providing exceptional care across various locations. This role is crucial in ensuring seamless patient care within the Med-surg/Acute Rehab unit.
Role Responsibilities:
- Collaborate with the healthcare team to create individualized care plans that prioritize patient safety and quality outcomes.
- Oversee adherence to care protocols, assess their effectiveness, and maintain ongoing communication with the healthcare team regarding patient progress.
- Empower patients in self-management through education and advocacy, fostering shared decision-making.
- Develop and implement policies that enhance patient care delivery.
- Contribute to the operational and capital budgeting processes.
- Assist in setting departmental goals and quality metrics, including data collection and reporting to relevant stakeholders.
- Work closely with the Director and nursing staff to manage daily staffing needs and create a 6-week staffing schedule.
- Facilitate continuous staff education, covering annual training, regulatory compliance, and patient safety initiatives.
- Exhibit effective leadership and drive continuous improvement within departmental activities, either directly or through delegation.
- Maintain up-to-date professional knowledge and skills.
- Demonstrate consistent and reliable attendance.
- Perform additional duties as necessary.
Qualifications:
Preferred: Bachelor of Science in Nursing (BSN).
Certifications: BLS, ACLS.
Work Environment:
Full-Time position with standard hours of 36.00. Evening shifts are required.
Essential Working Conditions:
* Manual dexterity and coordination required.
* Sedentary/light physical effort; may involve lifting up to 10 lbs.
* Exposure to various environmental factors.
