Role Summary:
The Clinical Coordinator is responsible for overseeing the continuity of care for patients within the Med-surg/Acute Rehab unit. This role involves managing a dedicated team that excels in critical thinking, clinical judgment, and effective communication with all members of the healthcare team. The Clinical Coordinator will leverage clinical expertise to enhance patient care and collaborate with the healthcare team to implement quality initiatives, performance improvement projects, and ensure compliance with regulatory standards.


Key Responsibilities:

  1. Collaborate with the healthcare team to create and implement individualized care plans for patients, ensuring safety and quality outcomes.
  2. Monitor adherence to care plans, assess their effectiveness, track patient progress, and maintain ongoing communication with the healthcare team.
  3. Empower patients and caregivers through education and shared decision-making, promoting self-management.
  4. Develop and enforce policies and procedures that guide patient care delivery.
  5. Contribute to the operational and capital budget planning.
  6. Assist in establishing departmental goals and quality indicators, including data collection and dissemination to relevant stakeholders.
  7. Work closely with the Director and nursing staff to manage daily staffing and develop a 6-week staffing schedule.
  8. Provide continuous staff education, covering annual training, regulatory requirements, and patient safety drills.
  9. Exhibit effective leadership and drive continuous improvement within departmental activities, either directly or through delegation.
  10. Maintain up-to-date professional knowledge and education.
  11. Demonstrate consistent and reliable attendance.
  12. Perform additional duties as necessary.

Qualifications:

Bachelor of Science in Nursing (BSN) is preferred.

Certifications: BLS, ACLS

Work Environment:
Full-Time position with standard hours of 36.00 per week, scheduled for evening shifts.

Essential Working Conditions:
Some manual skills and motor coordination required; sedentary/light effort with the ability to exert up to 10 lbs. of force; potential exposure to various environmental factors.
