Position Summary

As the Engagement Specialist, you will play a crucial role as the first point of contact for our organization, facilitating effective communication with employees, healthcare providers, patients, families, and external agencies. Your responsibilities will encompass a variety of tasks including reception duties, health information management, outreach for health assessments, and assisting with new patient enrollment.

Key Responsibilities

  • Welcome and assist all visitors and callers, including managing patient check-ins and scheduling follow-up appointments.
  • Handle the movement of medical records, including retrievals and health plan audits.
  • Update electronic health records with patient information and status changes.
  • Conduct Health Assessment Outreach for designated populations and arrange appointments.
  • Prepare and coordinate the delivery of supplies to clinic locations.
  • Serve as the primary contact for new patient screenings for home-based care versus clinic settings.
  • Ensure completeness of New Patient Packets, verifying insurance and eligibility while performing data entry.
  • Act as a liaison between the organization and patients/families for new patient enrollment activities.
  • Maintain communication between the Engagement Team and Primary Care Team regarding new patient admissions.
  • Follow up on all admissions and notify the primary care team of discharge dates.
  • Manage check-in and check-out processes for Home-Based Primary Care and clinic settings.
  • Keep the community resource database updated.

Qualifications

  • Strong verbal and written communication skills.
  • Exceptional interpersonal and customer service abilities.
  • Organizational skills with a keen attention to detail.
  • Proficient time management skills with the ability to meet deadlines.
  • Capability to prioritize tasks effectively.
  • Able to thrive in a fast-paced and sometimes stressful environment.
  • Familiarity with various health insurance plans, particularly those utilized within GSC.
  • High school diploma or equivalent required.
  • Proficient in English, with strong reading, writing, and speaking skills.
  • Ability to sit for extended periods and work on a computer.
  • Must be able to lift up to 15 pounds occasionally.
