Position Summary

The RN Case Manager is responsible for delivering direct patient care in alignment with physician directives, adhering to established standards of practice, and following company policies and procedures. This role involves implementing the care plan, providing treatments, and collaborating with the healthcare team to achieve optimal patient outcomes.

Key Responsibilities

  1. Conduct comprehensive assessments of patients and their families to identify hospice needs, including detailed physical evaluations and medical histories.
  2. Deliver skilled nursing care in the home setting, applying all elements of the nursing process.
  3. Regularly assess and evaluate patient status, adjusting the care plan as necessary based on ongoing evaluations.
  4. Plan, coordinate, and provide quality interdisciplinary home health services while adhering to company standards and regulatory guidelines.
  5. Ensure all necessary documentation is completed promptly and accurately, maintaining compliance with electronic medical record requirements.
  6. Initiate and modify care plans based on patient needs and conditions, involving patients and families in the planning process.
  7. Administer medications and treatments as prescribed, ensuring adherence to the physician's care plan.
  8. Educate patients and families on self-care techniques and health management as appropriate.
  9. Support patients with daily living activities, promoting self-sufficiency and comfort.
  10. Act as the Case Manager for assigned patients, coordinating care and communication among the healthcare team.
  11. Maintain accurate clinical notes regarding patient conditions and care provided, documenting any changes in symptoms or management outcomes.
  12. Communicate effectively with physicians regarding patient needs and condition changes, ensuring timely updates and necessary orders.
  13. Collaborate with community health professionals to coordinate comprehensive care plans.

Qualifications

Must be a graduate of an accredited nursing program, with a preference for a Bachelor of Science in Nursing (BSN). A minimum of one year of nursing experience is required, with hospice experience preferred. Knowledge in geriatric, chronic illness, restorative, and psychiatric nursing is advantageous.

Licensure and Certifications

A valid RN license in the practice state is required. Candidates must possess a valid driver's license and have access to a vehicle that meets state insurance requirements. Current Basic Life Support (BLS) certification for healthcare providers is also required.
