Job Overview

Position Summary:

The Admissions Registered Nurse plays a crucial role in managing patient information alongside the Admissions Coordinator, Admissions Assistant, and Community Liaisons. This position involves direct interaction with referred patients to conduct thorough assessments of their clinical conditions, which are then communicated to the Hospice Medical Director for either admission certification or denial. Upon receiving verbal certification from the Hospice Medical Director, the Admissions RN is responsible for coordinating all elements of the patient's admission, including necessary transportation, durable medical equipment, and medications pertinent to their hospice care. Additionally, the Admissions RN documents all relevant data in the patient's hospice medical record and ensures effective communication with the assigned hospice team to maintain continuity of care.

Key Responsibilities:

  1. Uphold patient rights and adhere to confidentiality policies.
  2. Collect, report, and analyze patient care data to formulate a comprehensive assessment for the Hospice Medical Director.
  3. Promptly report significant health or safety concerns to the relevant team members.
  4. Establish and maintain effective working relationships with peers and community personnel.
  5. Clearly explain hospice benefits to potential patients and their families.
  6. Collaborate with patients and families to develop a realistic Plan of Care and treatment goals upon admission.
  7. Supervise and delegate tasks to Certified Nursing Assistants and Licensed Vocational Nurses as needed.
  8. Anticipate the overall care needs of newly admitted patients and coordinate with all hospice disciplines.
  9. Ensure compliance with documentation policies and procedures related to patient admissions.
  10. Complete all necessary admission paperwork within 48 hours of patient admission.
  11. Exhibit sound judgment and decision-making skills.
  12. Participate in Interdisciplinary Team Meetings and other performance improvement initiatives.
  13. Provide emotional and spiritual support to patients and families, addressing concerns related to terminal illness.
  14. Facilitate communication among patients, families, and the hospice team.
  15. Identify caregiver strain and recommend respite care or support services as needed.
  16. Perform personal care duties as assigned by the Interdisciplinary Team.

Qualifications:

  1. Must possess a current Registered Nurse license in good standing with the State Board of Nursing.
  2. Preferred experience of at least one year in hospice, home health, or geriatric care.
  3. Must successfully complete a background check and pre-employment screening.
  4. Proficiency in reading, writing, and understanding the English language is required.
  5. Ability to manage time effectively and handle frequent interruptions.
  6. Must be available for on-call hours, weekends, and holidays.
  7. Reliable transportation and a valid driver's license are necessary.
  8. Current CPR/BLS Certification is required.
