Job Summary

We are seeking a highly skilled and compassionate Mental Health Clinician to join our team at Sound. As an Assisted Outpatient Treatment Clinician, you will play a critical role in providing intensive outpatient services to a small cohort group of older adolescents and adult mental health clients.

Key Responsibilities
  • Provide direct services and/or internal and external organization referrals for clients.
  • Develop treatment plans and maintain accurate and timely clinical records according to organization policy.
  • Perform outreaches/services to community locations to engage clients in care.
  • Comply with County and State standards by meeting service intensity expectations.
  • Prepare necessary reports, summaries, and progress notes, and incorporate them into clinical records.
  • Perform consultations and liaison duties with other agencies and resources on behalf of the client.
  • Assist clients to remain stable in the community, thereby reducing the use of hospitals or involuntary treatment services.
  • Monitor medication regimes and treatment compliance.
  • Monitor utilization of services and the success of outcome measures.
  • Participate in team member meetings, consultations, case presentations, and in-service training.
  • Participate in ongoing program development and assessment.
Requirements
  • Bachelor's degree in social work, psychology, or a counseling-related field, with a strong emphasis on working with families and community agencies.
  • A minimum of one year of clinical experience as a clinician or case manager and demonstrated skills in clinical assessment and treatment plans, preferred.
