Job Summary

We are seeking a highly skilled Mental Health Service Coordinator to join our team at Special Service for Groups- AP Recovery. As a key member of our multidisciplinary team, you will provide essential mental health and case management services to chronically mentally ill adults with complex needs.

Key Responsibilities:

  • Provide comprehensive mental health, case management, and substance use services to ensure continuous care coordination.
  • Assist clients with vocational, housing, self-help, and peer support services, including money management.
  • Coordinate services with public and private community health, mental health, and social service agencies.
  • Develop and maintain therapeutic relationships with clients and family members.
  • Participate in treatment recommendations and daily/weekly contact with clients.
  • Submit billing records and maintain client files and documentation in accordance with DMH guidelines.

Requirements:

  • Bachelor's degree in Psychology or Social Work or minimum 2 years experience working with mentally ill populations.
  • Spanish speaking preferred.
  • Reliable transportation, valid Driver License, and proof of automobile insurance coverage.
  • Basic computer software programs (e.g. Word processing, spreadsheet, and internet).

Estimated Salary: $45,000 - $60,000 per annum, depending on qualifications and experience.
