Job Summary

The Case Manager will provide mental health and case management services to chronically mentally ill adults with a history of non-compliance with treatment, including substance abuse, homelessness, and/or incarceration.

Responsibilities include:

  • Providing mental health, case management, housing/benefits assistance, vocational/socialization & recreation, substance use services, and other community resources.
  • Linking clients with necessary services and referrals.
  • Assisting with vocational, housing, self-help, and peer support services, money management.
  • Coordinating services with various public and private community health, mental health, and social service agencies.
  • Ongoing outreach and engagement with clients.
  • Reporting program activities and development, crises, and any other pertinent information to the appropriate chain of command.
  • Transporting clients to connect with necessary community resources.
Requirements

To be successful in this role, candidates must have:

  • A Bachelor's degree in Psychology/Social Work or related field, or at least 2 years of experience working with mentally ill populations.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Familiarity with community resources preferred.

This position requires reliable transportation, a valid driver's license, and proof of automobile insurance coverage. Salary is estimated to be around $40,000-$60,000 per year, depending on location and experience.
