Job Overview:

Salary Range: $65,000 - $80,000 per year

About the Role:
The Mental Health Care Coordinator will provide comprehensive mental health services to homeless individuals, including assessment, treatment planning, and coordination of care. You will work closely with a multidisciplinary team to ensure that clients receive the support they need to overcome obstacles and improve their coping mechanisms.

Responsibilities:
  • Conduct functional and strength-based assessments to identify clients' needs and develop personalized care plans.
  • Provide psychotherapy, counseling, and education using evidence-based interventions.
  • Coordinate care with primary care providers and other healthcare professionals to ensure seamless service delivery.
  • Advocate for clients within the organization and community to ensure access to appropriate care.

Requirements:
  1. Clinical Social Work Registration with BBS as an Associate Clinical Social Worker.
  2. Master's degree in social work from an accredited college or university.
  3. Experience working with low-income and homeless individuals.
  4. Demonstrated understanding of the impact of trauma on behavioral and medical conditions.
